Great pictures and good discussion and look forward to the final diagnosis. Based on the photos, my opinions are similar to many above: 1. High grade DCIS with focal central necrosis, cribriform growth pattern, with cancerization of lobules 2. LCIS 3. No invasive carcinoma is seen I wonder about whether there is any invasive cribriform carcinoma; but no irregular nests, no desmoplastic reaction, all DCIS confined within the TDLU (greatly expanded); fortunately the treatment for DCIS and DCIS with microinvasion is similar in most places.
