This is very interesting case with special cytomorphology. I notice the CK positive stain. The key is to make sure it is a carcinoma first. If it were my case, I would stain AE1/AE3, cam5.2, CK7. If it is carcinoma, I would stain E-cad and p120 to make sure if it is a lobular ca. Also will stain ER, PR, Her2. If it is lobular ca, it is pleomorphic or signet ring type. It is not a classic type invasive lobular ca. If it is not a lobular ca and if it is triple negative, I would stain ck14, ck17, ck5, EGFR to see if it is basal-like tumor. |
